Ya’ll ready to rodeo? I cannot believe it is almost time for our first rodeo! Where did summer break go? I don’t know about ya’ll but my family and I have been busy as ever! It has been great to be able to spend time with the people I love! I hope that you all have enjoyed your summer also! But, summer is indeed about over and now it is time to get back to school and for a new rodeo season to get started! I can’t wait to see you all at our first rodeo! I am so excited! Attached you will find a Membership Application, Release/Waiver of Liability & Indemnity Agreement, & W-9. Don’t forget that the release form HAS to be notarized. I NEED THE MEMBERSHIP APPLICATION AND RELEASE/WAIVER OF LIABILITY & INDEMNITY AGREEMENT FORMS BACK TO ME NO LATER THAN 9/1/10.

GA-FL Youth Rodeo Association

There are seven approved events in the GA-FL YOUTH RODEO ASSOCIATION and each one shall have a Student Director and Adult Assistant.

All events are open to teens from age 14-19. You must be at least these ages or between as of September 1 of that particular season in order to be an eligible member however should you turn 14 during that rodeo season, you will then be eligible to compete. If you are age 19 on Sept. 1 of that season, then you are allowed to rodeo that entire season. There will be only one (1) age division. A copy of your birth certificate must accompany your application unless you are a returning member along with the attached release and W-9. The W-9 is required for everyone but will only be issued for those earning over $599 for the taxable year.

Academic eligibility will not be required by the GFYRA. Therefore, you will not be due any grades at any point.

All contestants are required to call-in to enter their first rodeo. Call-in will be the Monday night before each rodeo. You will then be called in automatically at all rodeos for the remainder of the season. However, should there be any changes or additions to your riding status, you MUST call at designated time to make the necessary changes. Should you be a no show for a rodeo, you will be fined $15 per event called in for and will be remain ineligible to compete until said fine is paid. You will then be required to call back in to compete in future rodeos.

I have enclosed a Membership Application, Release/Waiver of Liability & Indemnity Agreement & W-9. Please return the enclosed forms along with your Membership Fee. The membership fee is itemized on an additional page. All new members (only) will need to submit a copy of their Birth Certificate.

The above listed do hereby desire to become a member of the GA-FL YOUTH RODEO ASSOCIATION. By making this application, it is understood that that a copy of each members birth certificate must be on file with the GFYRA secretary. The above member applicants do further understand that they will not be permitted to participate in any rodeo unless all information is on file and they are called in according to call-in rule.

We will abide by the rules and regulations of the GA-FL YOUTH RODEO ASSOCIATION and we will accept the decision of the Judges as final. We will conduct ourselves in a sportsmanlike manner at all times.

Please enclose the yearly Individual Membership Fee of $150.00 with this application. All forms must be completed entirely. If parents of member live at separate residences, please have both signatures and contact info. One application per contestant!

IN CONSIDERATION of being permitted to enter for any purpose and RESTRICTED AREA (herein defines as including but not limited to the arena, chutes, staging areas, approaches thereto and all walkways, concessions and other areas appurtenant to any area where any activity related to the even shall take place), or being permitted to compete, officiate, observe, work for, or for any purpose participate in any way in the event, EACH OF THE UNDERSIGNED, for himself, his personal representatives, heirs, and next of kin, acknowledges, agrees and represents that he has , or will immediately upon entering any of such restricted area, and will continuously thereafter, inspect such restricted area and all portions thereof which he enters and with which he comes in contact, and he does further warrant that his entry upon such restricted areas and his participation, if any in the event constitutes and acknowledgement that he has inspected such restricted area and that he fins and accepts the same as being safe and reasonable suited for the purposes of his use, and he further agrees and warrants that is, at any time, he is in or about restricted area and he feels anything to be unsafe, he will immediately advise the officials of such and will leave the restricted areas:

1) HEREBY RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE the promoted, participants, rodeo association, sanctioning organization or any subdivision thereof, arena operator, arena owner, officials or any persons in any restricted area, promoters sponsors, advertisers, owners and lessees of premises used to conduct the even and each of them, their officials or any persons in any restricted area, promoters, sponsors, advertisers, owner and lessees of premises used to conduct the event and each of them, their officers and employees, all for the purposes herein referred to as “releases”, from all liability to the undersigned, his personal representatives, assigns, heirs, and next of kin for any and all loss or damage, and any claim or demands thereof on account of injury to the person or property or resulting in death of the undersigned, whether caused by the negligence of the releases of otherwise while undersigned is in or upon the restricted area, and/or competing, officiating in, observing, working for, or for any purpose participating in the event.

2) HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releases and each of them from any loss, liability, damage, or cost they may incur due to the presence of the undersigned in or upon restricted area or in any way competing, officiating, observing, or working for, or for any purpose participating in the even and whether caused by negligence of the releasees or otherwise.

3) HEREBY ASSUMES FULL RESPONSIBILITY FOR AND RISK OF BODILY INJURY, DEATH, OR PROPERTY DAMAGE due to the negligence of releases or otherwise while in or upon the restricted area and/or while competing, officiating, observing, or working for or for any purpose participating in the event.

EACH OF THE UNDERSIGNED expressly acknowledges and agrees that the activities of the event are very dangerous and involve risk of serious injury and/or death and /or property damage. EACH OF THE UNDERSIGNED further expressly agrees that the foregoing release waiver and indemnity agreement is intended to be as broad and inclusive as is permitted y the law of the Province or State in which the event is conducted and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding continue in full legal force and effect.

THE UNDERSIGNED HAS READ AND VOLUNTARILY SIGNS THE RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT, and further agrees that no oral representations, statements or inducements apart from the foregoing written agreements have been made.
